Providers interested in receiving detailed information regarding practice
opportunities in rural and underserved communities in Colorado should
first complete the Provider Interview Form (click below to download the
appropriate Provider Interview Form in Word). Provider information will
be kept confidential and will not be provided to third parties until provider
approval has been received.
Provider Interview Form
Providers who are matched to an opportunity will receive detailed community
and practice profiles. These profiles contain information on recreation,
schools, community history, and county demographics. These profiles may
also contain links to the local Chamber of Commerce or city/county websites.
After reviewing the profiles, the provider should notify CPR of their
level of interest in the opportunity. CPR will facilitate communication
between the community and the provider.
CPR knows that providers also look for opportunities in other states.
The 3R Net website allows you to look for rural opportunities in states
other than Colorado. For more information on 3R
