Colorado Rural Mobile Health Project (CRMHP)
Salud Family Health Center is a Community Health Center with clinics in Brighton, Commerce City, Fort Lupton, Frederick, Longmont, Estes Park, Fort Morgan, Sterling, and Fort Collins. They have a mobile unit that provides immunizations – for children and adults, health screenings – medical and dental, mental health evaluations, and urgent primary care services. Patients seen on the mobile unit are referred to a Salud Clinic for follow-up care. The Unit provides a major outreach effort for hard-to-reach migrant and seasonal farm workers, and other vulnerable populations. The CRMHP grant funds were used to purchase a cholesterol machine, and the strips used for testing, and to purchase bilingual education materials to explain what cholesterol is and how it is used as an indicator to determine the patient’s risk for Cardiovascular Disease. The intent of the project is to detect high cholesterol in patients in rural areas who would otherwise go unscreened. The overall goal of the program is to reduce the risk of Cardiovascular Disease (CVD) by early detection and management of high cholesterol in a population at a much higher risk from both CVD and high cholesterol. Salud Family Health Center received $35,288.00.
