Colorado Rural Mobile Health Project (CRMHP)
Valley-Wide Health Systems, Inc. (VWHS) is a private, non-profit 501(c) (3) Community/Migrant health Center (C/MCH). Established in 1976, VWHS serves 18 counties in southern Colorado through a variety of programs including a Farmworker Health Service Program (FWHS) that provides vouchers to patients in areas of the state that do not house C/MHCs. Valley wide provides services in three distinct rural regions of the state (Arkansas Valley-AV, San Luis Valley-SLV, and the Southwest Corner) through 14 primary health and 6 oral healthcare sites. Valley-Wide Health Systems, Inc. (Valley-Wide) proposes to purchase a Mobile health Clinic unit to help expand and improve comprehensive healthcare services to vulnerable populations (described below), who are not currently linked to the traditional healthcare system. Target populations will include clients of the homeless shelter, correctional facility residents, seniors at the senior citizen center, agricultural warehouse workers and farm laborers, and residents of low-income housing units and head start sites. Services will focus on those at highest risk including the uninsured, underinsured, low-income, and medically underserved, with a particular emphasis for those in remote areas and/or with transportation barriers in Alamosa, Conejos, Costilla, Mineral, Rio Grande and Saguache. VWHS received $263,000.00.
