CREATE is Now Accepting New Applications!
There are several required steps before you can access the CREATE Application, and they MUST be completed in order for your application to be considered. Pay close attention to how and to whom each step is submitted--CRHC is not responsible for unevaluated applications due to the submission errors of applicants. Download the Grant Guidelines for detailed information on each step.
STEP 1: Contact Information Form
For each new request, you must fill out and submit the online contact information form in order to access the CREATE grant application. Your application will not be accepted until the form is received by CRHC. The information you provide in the contact information form will be used to communicate with you regarding your grant application. Once your Contact Information form has been submitted, you will receive log-in information for the grant application via email, within 72 business hours of receipt.
STEP 2: Agency Profile
All agencies applying for a grant must complete an agency profile. This includes facilities, non-transport agencies, community colleges and clinics. Complete or update the agency profile and submit it electronically using the CEMSIS Web portal. You must have a username and password to enter the portal. If you applied for a grant last year, or are submitting data to the MATRIX, you can use that username and password. If you need a new username and password, call 303.692.2990.
STEP 3: MATRIX Data Submission (Transport Agencies Only)
Transport agencies must complete a declaration of participation in MATRIX. Participation in MATRIX is mandatory for transport agencies. MATRIX data must be submitted for the previous quarter in order to be eligible for this grant.
Non-transport agencies are exempt from Step 3 and do not need to complete this step. MATRIX data is also submitted via the CEMSIS Portal.
STEP 4: W-9 Submission
All agencies applying for a grant must complete a W-9, even if one has been previously submitted. This includes facilities, non-transport agencies, community colleges and clinics. Download the W-9, complete it, and email to Lakesha Jones at email@example.com or fax to 303-832-7496.
