The list shows the name of the clinic, its city location, as well as whether it is provider-based or independent. "Provider-based" RHCs are affiliated with a parent entity which is typically a rural hospital or hospital system. To qualify as a provider-based RHC an established relationship must exist between the RHC and the affiliated provider. "Independent" RHCs are free-standing clinics, owned and operated as a single entity.
